【專刊】「務必告知患者之用藥訊息資料庫建置及資訊網推廣」
Establishment and Promotion of “Database of Essential Drug Usage Information for Patients” in Taiwan

目的:為使『優良藥事執業規範』能落實執行達成病人安全之目標,2007年建置具有迫切需要性的「務必告知患者之用藥資訊資料庫」,作為藥事人員進行患者用藥指導之參考,確保病人用藥之安全。
方法:引用適切的關鍵字,針對各大醫藥資料庫、EBM網站,進行系統性之文獻搜尋及參考十年內國內嚴重藥物不良反應通報、藥害救濟重大案例6,652件,並初步彙整研究藥品範圍,經專家委員充分討論後,確立「務必告知患者之用藥資訊」之定義並界定研究藥品類別。接續建立「務必告知患者之用藥資訊資料庫」、撰寫書籍、衛教單張之標準模式及校稿規範,確保資訊正確、完整與一致性。
結果:經專家委員共同研議,確立「務必告知患者之用藥資訊」乃指確保患者用藥安全務必知道之訊息,若此資訊未被告知,用藥結果可能導致患者重大危險。「務必告知患者之用藥資訊」內容涵蓋有用藥前、中、後注意事項與重要警語。為使「務必告知患者之用藥資訊資料庫」能切合民眾的需要並落實推廣與應用,更以民眾觀點切入,運用淺顯易懂的文字,配合活潑生動的警示插圖及用藥安全小錦囊,集結出版12大類371項藥品成分之用藥資訊書籍、用藥指導單張及建置「務必告知患者用藥資訊網」,並發展581項標準化的藥物不良反應中文名詞和59項用藥指導用語以及重要警語。
結論:提升醫療品質與病人安全是近年來世界衛生組織與全球各國醫療體系極度重視的議題與目標。「務必告知患者之用藥資訊」建立後,各醫療院所或社區藥局的藥事人員,可運用此資訊內容主動傳遞給患者。另一方面,民眾可透過閱讀「務必告知患者用藥資訊書籍與指導單張」或網站查詢,掌握正確用藥的重點,瞭解如何避免藥害以及當發生用藥問題時之適當處理方式,為自身「用藥安全」把關。

Aims: In order to enable GPP to carry out the execution to achieve goal of the patient safety, in 2007 establishes completes was of tantamount importance and necessity to establish a database on the “Essential Drug Usage Information for Patients”, which the pharmacotherapists can use as a reference for providing instructions to the patients.
Methods: We conducted a systemic literature search on the worldwide and nationwide reported incidences of severe adverse drug reactions and major cases reported by the Drug Relief Foundation of Taiwan. Experts in the field were invited to define “the essential drug usage information for patients” and decide on the extent of investigation, which included 12 categories covering 371 drug ingredients. “The essential drug usage information for patients” refers to vital information that patients must know in order to use the drug safely. It includes important notifications and warnings before, during, and after drug use, whose omission might result in major hazards to the patient.
Results: The drugs that “may possibly lead to severe adverse reactions” were given the first priority for inclusion in the publication. In order to enable the widespread utilization of “The Essential Drug Usage Information for Patients” database and meet the actual needs of the public, expert and editing committees were established. Opinions from health care professionals and the general public were gathered. The contents of the database and the materials to be published were standardized. Drug information booklets and instructional brochures covering 12 categories of 371 drug ingredients were written using layman’s language and from the general public’s perspective. Lively cautionary illustrations and tips on drug safety were incorporated. Standardized drug use instructional phrases and warning messages regarding adverse drug reactions in the Chinese language were also developed.
Conclusions: The list of drugs and categories will continue to expand in the future. The drug use instructional brochures will be published simultaneously, and the “Essential Drug Usage Information Website” will be periodically maintained and updated, thus establishing a complete and comprehensive information system. Practicing pharmacists may utilize the content of the publications and actively provide necessary drug usage information for the patients. The instructional brochures may also be printed out and delivered to the patients. Moreover, both the health care professionals and the general public will be able to access the information from the website.
