【原著】建置門診個別化用藥指導照會服務暨成效分析
Effectiveness of Developing an Individualized Electronic Platform for Outpatient Drug Counseling

目的:在國內忙碌的醫療環境下,根據統計分析,醫院藥師交付藥品及提供諮詢服務時間,每位病人平均花費35 秒鐘。為提升病人用藥安全及縮短花費時間,透過E 化平臺,提供病人優質的藥事服務,藥師能更清楚如何協助病人用藥,以達到最佳的藥物治療結果。
方法:跨科部整合與個人化之衛教模式,創新的資源整合將衛教量身化、藥矚照會簡單化、雙向回饋即時化、資訊透明化、作業方便化,以及綠能環保無紙化,透過電訪追蹤亦同時矯正病人用藥錯誤行為,全面化推行資訊系統建立滴水不漏個案追蹤。
結果:持續檢討改善的成果,用藥指導照會件數從2013 年277 件,2014 年2,870件到2015 年達2,964 件, 病人服藥正確率與順從性高達99.5%, 認知評估改善達87.1%。採用李克特滿分5 分量表,病人滿意度4.86 分,醫師滿意度4.87 分。個別化用藥指導照會服務2015 年榮獲SNQ 國家品質標章認證。
結論:個別化用藥指導照會平臺不僅有效提升服務量和衛教品質,同時也顯著改善病人與醫師對藥師執行用藥指導服務的滿意度。
 
Objective: Because of the busy nature of the healthcare business at a tertiary referral hospital, the time for drug counseling has been reported to be as short as 35 seconds for each patient. The present study aimed at investigating the effectiveness of establishing an electronic platform for drug counseling in enhancing patients’ understanding of medication use.
Methods: Through interdisciplinary and individualized counseling, the electronic drug counseling system was established with the intention of providing a simple,effective, interactive, transparent, and environmentally friendly platform for communications among patients, physicians, and pharmacists. Besides, telephone follow-ups were adopted as a supplementary measure for identifying patients who did not receive counseling and for detecting erroneous medication use.
Results: The total number of patients receiving counseling increased from 2,777 in 2013 and 2,870 in 2014 to 2,964 in 2015 after platform implementation. Moreover, the percentages of drug compliance and understanding of drug use were 99.5% and 87.1%,respectively. The overall satisfaction of patients was 4.86, while that of physicians regarding the performance of pharmacists was 4.87 on a Likert scale of 1 to 5. The system was also accredited by the Symbol of National Quality (SNQ) Committee,Taiwan, in 2015.
Conclusions: An electronic platform for drug counseling not only effectively enhanced the quantity and quality of drug counseling, but also significantly improved the levels of satisfaction for both patients and physicians.
