Valproic acid 為臨床上常用的抗癲癇藥物之一，偶爾會造成肝功能不良、血小板低下等不良反應，本案例係報告一位同時患有阿茲海默症及肝硬化的77 歲女性，因癲癇發作無法有效控制，在併用多種抗癲癇藥物包括valproic acid 後，發生肝功能不正常及血小板低下的情形，經停用valproic acid 後，肝功能及血小板數值隨即恢復，在排除其他疾病和可能的藥物影響，懷疑是valproic acid 導致的肝毒性和血小板低下。藉此提醒醫療工作人員於臨床上使用valproic acid 時，須注意病人可能發生不良反應的潛在危險因子及發生不良反應時的處置方式，以期將傷害減至最低，進一步減少醫療花費。
 

Valproic acid is one of the most common used anticonvulsants, it may sometimes lead to adverse  reaction, such as abnormal liver function and thrombocytopenia. This report shows a seventy-seven years old female, who was  comorbid with Alzheimer's disease and liver cirrhosis, and she was treated with multiple anticonvulsants including valproic acid because of intractable seizure. And then, she suffered from abnormal  liver function and thrombocytopenia. After stopping use of valproic acid, liver function and platelet counts were recovered immediately. We strongly suspected that valproic acid may cause such adverse reaction after ruling out any problem that can cause the same situation including concomitant diseases  and  possible  drugs. By the way, we want to remind  the healthcare workers  to take care over the risk factors that may lead to adverse reaction and the further management when they are using valproic  acid.
