【原著】長效口服藥臨床給藥頻率之符合度分析

在臨床上,我們常發現有些藥品的給藥頻率異於一般建議的頻率,因此,我們想了解院內藥物其臨床給藥頻率與建議給藥頻率不同的情形,並探究其合理性。由資訊室讀取目前院內使用中的所有口服藥資料,藥師主要以AHFS及Drugs為參考準則,篩選其建議給藥頻率為QD-BID的藥物,加以分析其目前在臨床上的使用情形,本研究收集2001年6、7、8月門診及住院處方做分析,評估結果顯示,多數藥物使用上乃符合建議給藥頻率,少部分藥品可能因醫師不恰當的處方形態,因而出現不符合建議給藥頻率的情形。未來應加強藥師的介入,對少數用藥頻率不當者,即時告知醫師作適當的調整;另一方面也須加強藥師的再教育,增加對某些藥物特殊用法的認識,最終的目的乃期望改善整體醫療品質,使藥物發揮最大藥效。

Clinically, we often observe that the use of some drugs does not conform to the recommended dosing frequency. The objective of this study was to investigate the degree of discrepancy between the clinical and the recommended dosing frequency in a regional teaching hospital. The data used in this study was acquired from the information office of the hospital. Based on AHFS and Drugs, pharmacists selected oral drugs with the recommended dosing frequency of QD and/or BID, and then analyzed the clinical use of these drugs. Both outpatient and inpatient prescriptions in a three-month period (June-August 2001) were collected and analyzed. The results showed that the clinical use of most oral drugs adhered to the recommended dosing frequency. However, the use of few oral drugs–partly due to doctor’s inadequate prescribing patternsdid not follow the recommended dosing frequency. In conclusion, this present study on one hand suggested an important role of pharmacists to detect and report inadequate dosing frequency; on the other hand, this study suggested that pharmacists may need to reinforce their knowledge of the special use of some oral drugs.
