美 國 臨 床 藥 學 學 會 ( AmericanCollege of Clinical Pharmacy, ACCP)於1997年首次出版了由藥師執行「共同合作的藥物治療管理」(Collaborative DrugTherapy Management, CDTM)之聲明文件 。 美 國 醫 院 藥 師 協 會 ( American Society of Health System Pharmacists, ASHP)對CDTM的定義為:這是一種跨領域學問的過程,目地是為了提供並選擇適當的藥物治療、教育病人用藥安 全、監測病人疾病狀況、和持續不斷地 評估治療的結果。CDTM 包括以下這些 執業範圍:1.進行修改和監測病人藥物 治療劑量,2.開立和執行相關的醫療檢驗醫囑,3.評估病人對治療的反應,4.諮詢和教育病人對藥物的使用,5.提供藥物給病人使用等。CDTM是以一個團隊的方式去進行醫療照顧,藉著適當的藥物使用與加強病人治療達到理想的病人照顧結果,讓藥師與醫師或其他處方開立者將其專業發揮到最大。根據實際執行過 CDTM的經驗,此藥物管理計畫可以達到以下目的:1.縮短給藥方式的修正時間,2.減少不必要的醫師門診次數,3.增加病人的服藥配合度和對藥物治療的遵醫囑性,4.減少了避免因藥物治療所引起的其它問題等。
The American College of Clinical Pharmacy published its initial position statement on collaborative drug therapy management (CDTM) by pharmacists in 1997.ASHP (American Society of Health System Pharmacists) defines CDTM as:it is a multidisciplinary process for selecting appropriate drug therapies,educating patients, monitoring patients,and continually assessing outcomes of therapy.CDTM may include but is not limited to: initiating,modifying,and monitoring a patient’s drug therapy;ordering and performing laboratory and related tests;assessing patient response to therapy;counseling and educating a patient on medications;and administering medications.CDTM is a team approach to health care delivery. It maximizes the expertise of the pharmacist and the physician or other prescriber to achieve optimal patient care outcomes through appropriate medication use and enhanced patient care services. Experience with CDTM has shown a reduction in delays in modifying drug regimens,a reduction in unnecessary physician office visits, increased patient compliance and adherence to drug therapy plans,and an increased likelihood that drug therapy problems will be averted.
