目的:為提升用藥安全並強化常備藥品之管理,本研究建置醫院常備藥品e化管理系統,並訂定查核正確率達90%為目標,以評估本系統對常備藥品管理效率之提升。
方法:利用網頁資料庫技術建置本系統,每年分四季進行查核,分別計算常備藥品之品項、數量與效期三項評估指標正確率。此外,針對藥品異動與效期預警通知功能計算更換率進行成效分析。
結果與討論:利用本研究所建置的「常備藥品e化管理系統」,成功地將原先人工紙本記錄方式轉變成網路化資料庫管理模式。經分析98年第一季至第四季平均三項評估指標正確率分別為88.37%、95.89%、96.45%與98.14%,各項正確率逐季提升,從第二季至第四季均已達到設定目標值,表示本系統可有效控管全院常備藥品。在自動化E-mail發信通知機制中有67%的成功更換率,再配合電話輔助可達100%更換率。
結論:本研究建立之管理機制對醫院常備藥品之用藥安全提供以下貢獻:(1)促本院常備藥品管理效率化並針對缺失立即改善;(2)藥品異動主動搜尋通知,防止呆藥產生;(3)效期預警通知以提升藥品使用安全,並避免過期損耗。
Objective:
The aims of this study were to build an e–based system of standing drugs and to evaluate its efficacy in management.
Material and Methods:
We selected the internet technology to build this system and calculated the accuracy of item, count and expiration date in each department. Additionally, the replacement rate was counted for the e-mail functions of drug change and expiration date.
Results and Discussion:
This system has successfully transformed traditional paper work into web-based mode in the management of standing drugs. In 2009, the accuracy was further improved in the sequential four seasons (88.37%, 95.89%, 96.45% and 98.14%) to show that standing drugs were effectively controlled by this system. The replacement rate with e-mail notice was 67% and got up to 100% after the assistant of telephone call.
Conclusion:
This study provides the following contributions in medication safety: (1) to be more effective management of standing drugs, (2) to avoid dull medicine using e-mail notice when drug change and (3) to promote medication safety by expiration alarm message.
