【原著】比較團體衛教與個別指導對定量噴霧劑使用的影響

背景:吸入劑型的使用指導是臨床藥師重要的工作之一。研究顯示:住院病患給予一次的定量噴霧劑(MDI)個別指導,對其吸入技術有一定程度的幫助。然而一對一的教導相當費時,若能以團體衛教取代,相信能節省藥師人力。但是團體衛教的成效如何並未評估過,因此本研究計劃比較團體衛教和個別指導對定量噴霧劑使用技術的影響,藉以做為日後指導的模式。
方法:本研究將68位病患隨機分成團體衛教組和個別指導組,比較二組病人在定量噴霧劑指導前後技術得分之差異。同時也紀錄衛教花費時間。
結果:個別指導組36人之平均指導前技術得分為6.1  1.8(滿分10分),指導後顯著的進步成9.1  1.1(p = 0.000);團體衛教組32人之平均指導前技術得分為5.8  2.1,指導後顯著的進步成8.9  1.3(p = 0.000)。二組病人的技術進步分數分別為3.1  1.5分和 3.2  1.8分,二者無統計上顯著差異(p = 0.748)。然而團體衛教組平均指導一位病患所費工時只有個別指導組的四分之一。
結論:本研究的資料顯示:提供住院病患定量噴霧劑的團體衛教服務,其效果與個別指導相當,而且比較省時。
 
Backgrounds: Patient education on inhalation technique is one of the most important works of clinical pharmacists. Our previous study showed that personal counseling on metered dose inhaler (MDI) improved inhalation technique of inpatients. Nevertheless, personal counseling is time-consuming. While group instruction on inhalation technique may be time-saving, its effectiveness has not been evaluated. The aim of this study was to compare the effectiveness between personal counseling and group instruction on the use of MDI.
Methods: A total of 68 patients were randomized into two groups: personal counseling and group instruction. Inhalation technique of individual patients was assessed before and after instruction by means of checklists in each group. Time spent on instruction was recorded as well.
Results: The inhalation technique score before instruction was 6.1±1.8 (perfect score was 10) and significantly improved to 9.1±1.1 after instruction in the personal-counseling group (p = 0.000). In the group-instruction group, the scores before and after instruction were 5.8±2.1 and 8.9±1.3 respectively (p = 0.000). There was no significant difference between the two groups on the score of improvement, 3.0 vs. 3.1 (p = 0.748). Time spent on group instruction was only one forth that of personal counseling for each patient.
Conclusions: The data of this study suggest that group instruction is comparable to personal counseling on the improvement of MDI inhalation technique, while less time-consuming.
