【原著】臺灣社區藥師執行用藥配合度諮詢服務的成果
The Outcomes of Adherence Counseling Service Provided by Community Pharmacists in Taiwan

目的:藥師調劑後的用藥指導僅有3 分鐘告訴病人用藥資訊,若要確實了解病人在家是否按時吃藥或對用藥知識的正確性,藥師應多花時間執行用藥配合度諮詢服務。此研究主要探討社區藥師執行用藥配合度諮詢服務之成果。
方法:本研究是以藥師實際執業為導向的觀察性研究,由藥師公會全聯會規劃與執行培訓課程教導藥師執行專業服務,建立網路化電腦管理系統紀錄服務內容並分析服務成果。此諮詢服務是藥師針對病人服用每一項藥品,以五個開放式問句來詢問該藥的五大用藥認知,鼓勵病人說出自己認知與實際用藥情形,從中判斷病人的瞭解程度並找出錯誤所在,針對錯誤地方做教育。
結果:四個月執行服務期間共214 位藥師服務了2,697 位個案,接受服務的個案共使用10,998 項藥品,其中88.4% 的用藥有錯誤認知或行為,需藥師提供教育來導正其錯誤。藥師共教育個案31,678 個用藥知識,平均病人每正服用一個藥品就有2.9項認知或行為錯誤需加強輔導。
結論:由於許多用藥知識病人不知道或認知錯誤,藥師提供配合度諮詢服務有很大的病人需求量。此服務能針對病人用藥錯誤的認知或服藥行為而加以教育指導,改正用藥觀念與行為,提升病人在正確時間以正確劑量及方法服用正確藥物。
 
Objective: After dispensing, pharmacists usually spend 3 minutes advising patient on drug usage information. In order to understand correctly the drug taken knowledge and behavior of the patient, pharmacist should spend more time conducting counseling service. The study purpose was to build the pharmacist practice model and evaluate the effectiveness of providing counseling service.
Methods: The adherence counseling service model was developed by the Taiwan Pharmacist Association (TPA) with five open-ended questions to be asked for each medication the patient is taking, to induce more responses by the patient of their knowledge and drug taking behaviors at home. Pharmacists were recruited and trained to practice and every response from the patients should be recorded in the computerized documentation system developed by TPA. Patients who take refill prescription to the community pharmacy can be recruited in the study.
Results: Total of 214 pharmacists participated and provided adherence service to 2,697 patients. Total 10,998 medications were counseled, 88.4% of which patients had wrong knowledge or took the drug inappropriately that required pharmacists to provide proper education. Pharmacist will ask 5 questions on each drug, and total of 31,678 behavior changes were educated. In average, for each medication patient is taking, 2.9 drug use information were wrong and needed correction.
Conclusions: Adherence counseling provided by pharmacist has great need in the society. The service identified inappropriate knowledge and drug taking behavior and provides individualized patient education try to increase the proper usage of those medications and hopefully to reach the goals of right drug to the right patient at right dosage/route and right time.
