【案例報告】鋰鹽與Indapamide藥物交互作用導致嚴重的鋰鹽毒性:案例報導及文獻回顧

鋰鹽是一種可有效控制躁鬱症的藥物,過去文獻較少報導老年人併用鋰鹽與indapamide 產生藥物交互作用的嚴重後果。本文案例將分享老年病人因併用鋰鹽與indapamide 約莫月餘,發生藥物交互作用而導致鋰鹽中毒的事件。一位62 歲的男性病人因為併用indapamide 與鋰鹽將近一個月的時間,而導致致命性的鋰鹽中毒現象。老年人、多重藥物使用、腎功能異常、糖尿病、高血壓、心衰竭、肝硬化、腸胃炎、限鈉飲食及脫水等皆可能為導致鋰鹽中毒的危險因子。因此,在臨床上,醫師及藥師皆需特別注意這些鋰鹽相關的藥物交互作用,並頻繁監測病人腎功能及鋰鹽血中濃度以減少中毒反應。

Lithium is an effective medication for maintenance treatment of bipolar disorders. Limited reports related to a thiazide diuretic-lithium interaction in the older persons are noted. We present a case of lithium intoxication secondary to interactions between lithium and indapamide. A 62-year-old man developed lithium toxicity after concurrent use of indapamide for approximately one month. Risk factors for lithium toxicity include being older persons, polypharmacy, renal impairment, diabetes mellitus, hypertension, congestive heart failure, cirrhosis, gastrointestinal (GI) illness, sodium-restricted diets and dehydration. Clinicians should be aware of these harmful lithium-drug interactions and monitor the renal function and the daily serum lithium concentration to reduce the risk.
