【原著】某醫學中心住院跌倒病人用藥分析
跌倒、致跌藥物使用、跌倒發生率、Fall, Fall-Alert Medications, Incidence of Falls

目的: 評估病人用藥與跌倒事件發生的相關性。
方法: 採回溯性病例對照研究 (retrospective, matched case-control study),自2011 年7月1日起至2013 年6月30 日入院符合收案條件之病人,藉由病歷與跌倒事件報告,收集病患基本資料及使用之藥物,以羅吉斯迴歸(logistic regression) 分析病例組與對照組間之藥物使用是否具顯著差異。
結果: 病例組 (n = 211) 及對照組 (n = 592) 共803 人,以使用抗憂鬱藥者 [ 跌倒組23 人 (10.9%)、非跌倒組34人 (5.7%)] 與使用類固醇者 [ 跌倒組69 人(32.7%)、非跌倒組115 人(19.4%)] 在統計上有顯著差異。
結論: 研究結果顯示,使用抗憂鬱劑或類固醇者會顯著增加兩倍住院病患跌倒的風險,需優先介入評估用藥,及早落實防範病患跌倒措施。
 
Objectives: This study investigate the association between medications usage and probabilities of fall and to identify medications that may contribute to the risk of falls in inpatient setting.
Methods: Restrospective case-control study with propensity score design was conducted in a medical center in central Taiwan. Data were collected from nursing records and medical records,including adult patients (≥ 20 years of age) admitted between 1 July 2011 and 30 June 2013.Logistic regression was used to explore the relationship of medication usage and probabilities of fall.
Result: Of the 312 documented fall events in two year period, 211 patients met the inclusion criteria with 592 of control patients matched the criteria. The probabilities of fall significantly increased when the patients used antidepressants (adjusted odds ratio [AOR] 2.002; 95% CI: 1.15, 3.487; p =0.014) and corticosteroids ([AOR] 2.023; 95% CI: 1.422, 2.879; p < 0.01).
Conclusion: The use of antidepressants and corticosteroids demonstrate a significant association with falls in this population. Further researches with sufficient sample and multiple institute settings are needed to confirm the findings.
