本文旨在介紹衛生福利部二年期藥師訓練課程,新進藥師之可信賴專業活動 (entrustable professional activities, EPAs) 共識形塑的歷程。由來自全國27 家教學醫院臨床教師聚焦討論,以名義團體法進行共識及投票,針對八項新進藥師需達到信賴授權之臨床任務,包含「門診處方評估」、「門診處方藥品交付」、「門診病人藥品諮詢」、「藥品不良反應評估」、「住院病人用藥評估」、「療劑監測評估與建議」、「醫療人員藥品諮詢」、及「管制藥品調劑與管理」,歷經四次各半天的共識討論,確認此八項EPAs 其任務主題、對應的任務內涵及其限制之代表性與適切性。透過建置二年期藥師核心可信賴專業活動,可應用於醫療職場教學訓練,以信賴等級的評定,進一步轉化為工作職場上的監督與授權等級,此八項EPAs 的建置,為臺灣藥師執業訓練奠定基礎,期待以此開啟勝任能力導向教育,精進藥學專業執業之品質。
 
This article aims to introduce the process of shaping a consensus on entrustable professional activities (EPAs) for newly licensed hospital pharmacists in the two postgraduate years (PGY) training program sponsored by the Ministry of Health and Welfare. Twenty-seven clinical educators from teaching hospitals nationwide gathered to reach a consensus via the nominal group technique (NGT) on eight clinical tasks that newly qualified pharmacists should be trained on and entrusted with. These tasks include “outpatient prescription verification”, “outpatient medicine delivery”, “patient medication counseling in outpatient settings”, “adverse drug reaction evaluation”, “inpatient medication use assessment”, “therapeutic drug monitoring evaluation and recommendation”, “medication consultations with healthcare professionals”, and “dispensing and management of controlled drugs”. After four half-day-long discussions and consensus processes, the representativeness and validity of the thematic areas, the content of these tasks, and limitations of these eight EPAs were confirmed. By establishing the core EPAs for two-year PGY pharmacists, we could apply them in teaching and training at clinical workplace to determine the levels of entrustment through a supervisory hierarchy, which are further translated into clinical privilege. The establishment of these eight EPAs lays the foundation for pharmacist training in Taiwan, with the expectations that competency-based education will strengthen the quality of pharmacy practice.
