Caspofungin 是第一個上市的 echinocandin 類抗黴菌藥，廣泛的應用於成人侵入 性麴菌和念珠菌感染的治療，但 caspofungin 並未被核准用於 18 歲以下的青少年及 兒童，小兒的臨床使用經驗非常有限，尤其是新生兒和早產兒。我們報告一個早產 兒持續性念珠菌血症的案例，使用 fluconazole 治療失敗，並且無法耐受 amphotericin B 的副作用，我們以 caspofungin 治療  18 天，最後成功的根除黴菌，感染的臨床症 狀和血液檢驗數值都獲得改善。以 caspofungin 成功治療早產兒持續性念珠菌血症， 這是國內首次有這樣的報告。

Caspofungin is the first echinocandin antifungal agent widely used for the treatment of adult invasive aspergillosis and candidiasis. However, it had limited experience in pediatrics, especially in neonates and premature. We present a case of premature with persistent candidemia, who failed to fluconazole therapy and could not tolerated adverse effects of amphotericin B. He received caspofungin therapy for 18 days. Finally, the fungal was eradicated. Clinically signs and laboratory tests were improved. This is the first report about caspofungin treatment in premature with persistent candidemia in our country.
