目的： 本研究利用國家衛生研究院全民健康保險研究資料庫之大型樣本，探討臺灣懷孕婦女使用處方藥之平均天數、處方藥品類別及其安全等級。
方法： 採用健保資料庫2005 年承保抽樣歸人檔，以ICD-9-CM 診斷碼篩選出於2005 至2007 年懷孕個案納為研究樣本，將其懷孕期間所有用藥，逐一進行藥理作用分類及安全級數編碼及統計。本研究之樣本納入原則為懷孕診斷起始日之後270 日內有生產診斷者。經篩選後共有1,586 人次符合並納為研究樣本。
結果： 經分析結果，懷孕期間之用藥類別，最多之前三名依序是「咳嗽及感冒製劑」(16.71%)、「解熱鎮痛劑」(11.60%) 及「制酸劑，抗逆流製劑及抗潰瘍藥」(10.73%)。在懷孕用藥安全等級方面，以B 級藥品最多（占62.79%），其次為C 級 (27.54%)、A 級 (7.29%)、D 級 (1.43%) 及X 級 (0.95%)。
結論： 臺灣婦女懷孕期間用藥以安全性較高的B 級藥品居多，但有少數使用高危險之D 級及X 級藥品，為不可忽視之用藥風險。
Objectives: This study used the massive samples of National Health Insurance Research Database (NHIRD)to explore the average days pregnant women used prescribed medicine and the category and safety level of the prescribed medicine they used during pregnancy.
Methods: A random sample was retrieved from Year 2005 NHIRD. A diagnosis code ICD-9-CM was used to screen pregnancy cases from 2005 to 2007 as research samples and then the medicines used by the selected samples during pregnancy were identified. The FDA pregnancy category was used to sort out the safety level of each medicine and the medicines were coded. This study only included the cases giving birth within 270 days from the first day of pregnancy. In total, there were 1,586 valid samples.
Results: The analysis showed the three most commonly used medicines during pregnancy were Cough & Cold Preparations (16.71%), Analgesics & Antipyretics (11.60%) and Antacids, Antireflux Agents & Antiulcerants (10.73%). Furthermore, the analysis on the safety level of pregnancy medicines showed Pregnancy Category B medicines were the most frequently used (62.79%),Category C the second (27.54%), Category A the third (7.29%), Category D the fourth (1.43%) and Category X the fifth (0.95%).
Conclusions: This study found that there was a “prenatal diagnosis” category coded “ICD-9-CM 640-677” in the NHIRD. During pregnancy, the most commonly used medicines were Pregnancy Category B medicines which were recognized as “probably safe.” However, a very few pregnant women used highly risky Pregnancy Category D and Pregnancy Category X medicines, the high risk of those drugs should not be ignored.
