【原著】公立醫院藥品聯標創新採行最有利標之執行成效分析
The Cost-Effectiveness Analysis of Adopting the Most Advantageous Tender in the Pharmaceutical Procurement of a Public Hospital

目的:為確保用藥安全、用藥品質,及病人用藥權益,臺北市立聯合醫院在符合政府採購法相關規定前提下,首創以最有利標決標方式辦理藥品聯標,本研究預定探討藥品聯標採行最有利標決標之執行成效。
方法:收集公開招標最有利標決標方式進行招標作業之評選作業方式、訂定評選項目、決標方式,並以描述性統計,比較藥品最有利標與歷年藥品最低標之開標次數、決標率、耗費藥師人日數、決標之藥品品質等。
結果:在本標案中,公告招標1,196 項藥品,計1,097 項藥品決標,決標率達91.7%;藥品資料審查耗費作業人力約76 人日數;決標藥品分類占率(決標廠牌項數/招標總品項數),成分專利期內藥品占率6.4%,過期研發廠藥品42.2%、BA /BE 藥品25.8% 及一般學名藥46.4%。決標品項屬目前使用中廠牌佔率共89.6%。
結論:經由藥品最有利標的評選機制,增加學名藥與原廠藥品的選擇,也提升了現行使用廠牌的延續性,減少民眾換藥困擾,雖決標未必是價格最低之廠牌,但透過各項公平公正評選項目,決標對醫院及病人最有利之廠牌,對於用藥品質的提升及用藥權益之確保,咸認具有重大之意義與效益。
 
Objective: To advocate the drug safety, quality, and more brand/generic choices,Taipei municipal hospital pioneered the adoption of the most advantageous tendering method (MAT) under the Guideline of the Government Procurement Act in its pharmaceutical procurement. This study was intended to analyze the costeffectiveness of adopting the MAT in the drug procurement process.
Methods: The reports on the implementation of MAT in the public tender was gathered and analyzed. Descriptive statistical analysis was used to compare the outcome of the MAT with the lowest tender in aspects such as the number of open tender hosted, the tender award rate, the cost of pharmacist manpower, and the quality of contract-awarded medication.
Results: There was a total of 1,196 objects listed and 1,097 tender awarded. 91.7% contract awarding rate was achieved with only 76 work hours used. The tenderawarded medication can be categorized (the number of tender-awarded medications/total number of tender items) as patent drugs (6.4%), patent-expired brand drugs(42.2%), generic drugs with bioavailability/bioequivalence qualification (25.8%) and regular generic drugs (46.6%). 89.6% of the contract-awarded brands were already in use.
Conclusions: Adopting the MAT method in the drug procurement process resulted in more drug choices available and reduced the chances of drug change. The MAT through fair selection process ensured tender award was not determined solely on price but also on quality. This new adoption of MAT represented a significant change that can increase the quality and the quantity of drug brands available in the hospital.
