【原著】條碼技術在臨床給藥之設計與運用
Design and Application Bar-Code Technology in the Clinical Administration

背景:近年來,由於醫療疏失事件不斷重覆上演,使得「病人安全」話題愈受討論與重視,各界不斷思考研究藉著科技技術之導入,來營造更安全且值得信賴的給藥方式,本報告旨在說明基於病人用藥安全之加強下,有關條碼核藥系統之設計與應用。
方法:將條碼技術導入藥品自動配方系統,配合無線網路傳輸醫師處方,便於藥師調劑與核對處方,也協助護理人員給藥時,核對藥名、劑型、劑量、服藥時間等。整個系統設計區分為硬體建置與軟體開發,硬體建置階段包括:藥品自動配方機體內控制系統晶片更換與修正、無線網路架設、平板電腦、掃瞄器購置等。軟體開發分為三大部分:一、微軟 Sql 資料庫(MS Sql Server):負責儲存與整理資料。二、網路伺服器(Web Server):負責傳送資料,主要利用 Windows 2000 Server、IIS
6.0、Asp.net 與 MS Services 來完成。三、使用端(user site,使用 Internet explorer):
(一)藥師調劑系統(二)護理人員給藥系統(三)電腦通報及管理者系統。
結果:藥品自動配方系統列印出「單一劑量餐包」專屬條碼,並結合條碼核藥系統,透過無線網路傳輸處方進行核對。初期採用人工進行處方內容逐筆核對,直到處方內容正確無誤後,於 2005 年 8 月起實際進行藥師調劑作業測試、護理人員
給藥測試各 10 次,並於 2004 年 6 月進行「系統使用前」使用者滿意度調查,2005
年 8 月與 2006 年 3 月進行「系統使用後」使用者滿意度調查。護理人員整體滿意度上升約 21%。系統使用者不滿意的原因主要有:無線網路品質不穩定、行動護理車電池續航力不足、系統操作不熟悉、操作比較繁雜等。
討論:利用資訊科技導入醫療作業中,將是未來的趨勢。我們相信條碼核藥系統之導入,能在護理人員給藥時,達到提醒、警示與二次核對之功能,預防人為用藥疏失並提高以「病人安全」為中心的照護品質。但是在導入資訊科技時,系統設計如何能達到「病人安全」的目標且又符合醫護人員的需求,將是未來面臨的挑戰。

Background: More attention had been placed on 「 patients safety 」 in the past few years which because medication error events unceasingly repeats the performance. All walks of life unceasingly pondering and researching one affiliation from the science and technology, to building safer also is worth trusting the medicine giving. This report is for the purpose of showing based on the patient strengthening of with under the medication safety, design of and application the related bar code administration system. 
Materials: Using bar code technology lead-in the Automated Medication Dispensing Systems, combining the wireless network transmission with prescriptions, convenient for the pharmacists to dispensing and checkup, and also serve as an valuable tool to check drug names, dosage forms, doses, timing, appearance and even the name of patients for nursing staffs during medication administration. Overall system designed for divides the hardware and the software, the hardware establishes the stage is: Exchange the control system chip and revision Systems formula organism in the Automated Medication Dispensing, erecting the wireless network, buying the tablet PC, buying the mobile nursing station, the scanner and so on. System software including three parts : 1. MS-Windows Sql Server : Saving and Collating dates 。 2. Web Server : Transmission dates , The most part using Windows 2000 Server 、 IIS 6.0 、 Asp.net and MS Services 。3. User site: Function for user (Just now using Internet explorer) including three parts: (1) Pharmacist Dispensing System, (2) Nurse Administering System, (3) Analysis and Reporting System.
Results: The drugs auto formulation system prints out the exclusive bar code of "single quantity the meal wrap", and combines the bar code pit medicine system, delivering the prescription to carry on check through the wireless network. Adopted the artificial to carry on the prescription content to pursue the pen to check in the early years, until the prescription content after ising right without any error physically carried on the medicine teacher to adjust a homework test, nursing from August of 2005, the personnel tested 10 for each for medicine, and carried on" the system usage front" user satisfaction's investigate in June of 2004, August of 2005 and carry on" the system usage behind" user satisfaction's investigate in March 2006.The personnel's whole satisfaction rising of the nursing is about 21%.The reason that the system user be dissatisfied with mainly has: the wireless network quality is unsteady, the continuous sail power of the action nursing car battery is not enough, the system operation does not acquaint with, operate more complicated etc..
Conclusions: Bar Code is a top information technology priority for hospitals in the next years. We believe the “Drug checking system” can be more effective to remind 、warning and double check while nurses administration, intercept medication errors than traditional medication delivery process, and furthermore improve patient medication safety. But inducting the science and technology into medical, how to balance in the system including 「 patient safety 」 and the medical provider demand will be the challenge which at the future.
